Annual Update Forms

ALL STUDENTS & PARENTS

The following forms must be printed out, completed and turned in at registration walk-through by ALL families:
  • Student Accident/Health Insurance Letter & Form (Spanish version) (1 per student)
  • Computer Use form?
  • Parent Code of Conduct ?
  • Photo Release Form???
 
5th Grade Students Only
If your child(ren) are in 5th grade, please print out, complete and turn in the 5th grade Healthy Kids Survey Parent Permission Form (1 per 5th grade student)

The following are for your reference only. No action is required.
  • 2014-2015 Instructional Calendar
  • Missing First 3 Days of School Notice
  • Student Acceptable Use Policy for District Digital Network

STUDENTS WITH SPECIAL MEDICAL NEEDS

If your child(ren) have any special medical needs, please print out, complete and turn in the form(s) below:
  • Medication at School Form (Required for any student who needs to have medications at school; 1 per student)
  • Parent Request for Emergency Treatment (Required for any student with a life-threatening medical problem; 1 per student)
  • Medication and Emergency Health Care Plan for Life-Threatening Allergies (Required for any student who has a life-threatening allergy; 1 per student)

If you would like to enroll or apply for any of the following programs, print out, complete and turn in the form below at registration walk-through:
  • Student Accident & Sickness Insurance Program Enrollment Form (1 per student)
  • Free/Reduced Lunch Program Application (Must meet qualification requirements; 1 per student)
